Every couple has a unique story, and their ceremony should reflect who they are, the love they share, and the desires they have for their special day. Having officiated well over 200+ weddings, I have learned that taking the time to get to know the couple makes all the difference. With that in mind, I want to share a simple wedding questionnaire I have developed to help you better understand the couple you are marrying and craft a ceremony that is personal, heartfelt, and something they will treasure for years to come.
Instructions for Completing the Fillable Wedding Questionnaire
This questionnaire is a fillable PDF, allowing the couple to type their answers directly into the form.
Download the Form: Download the Wedding Ceremony Questionnaire PDF to your computer.
Send It to the Couple: Email the PDF to the couple with instructions for completing and returning it.
Save Before Completing: Have the couple download and save the PDF to their computer before filling it out. They should open the saved file rather than completing it in their email or web browser.
Complete the Questionnaire: Ask the couple to take their time answering the questions together, providing as much detail as they feel comfortable sharing.
Save with a New File Name: Once completed, they should save the PDF using their names in the file name (example: John_and_Mary_Wedding_Questionnaire.pdf). This helps ensure their responses are saved and easily identified.
Email the Completed Form: Finally, have the couple attach their newly saved PDF to an email and send it back to you.
*Important: Remind the couple to reopen their saved PDF before sending it to make sure all their answers were saved correctly.
